I'll qualify with education + work experience

—Complete an Assessment Matrix — 3 combined years of education / work experience

—Have your supervisor complete a competency assessment form

—Transcripts (if using college education) · training certificates (if using CEUs)

—Supervision hours are primary-source verified by MHACBO
